Note on Online Documents

Online documents are published as PDFs. There are two formats available for most documents: (1) a printable document, and (2) a “fill-in” form. To use a “fill-in” form you must download it to you local computer or device. Once it is completed, e-mail it to the appropriate staff person.

New Parish Member Registration

Every new parish member is asked to complete the full Parish Membership Registration form. Only one form needs to be completed per family.

• Blank forms are available at the church.
  • When completed, drop the form off at, or mail to, the parish office.
    
• Printable Document
  • Download the PDF document and print it out. When completed drop it off at, or mail to, the parish office.
• Fill-in Form
  • E-mail the completed PDF Fill-in form to the Administrative Assistant.

Use one of the following methods to submit your contact information:

• Call, write or phone the parish office.
  • Our contact information is available on the “Contact Us” page.
• Use the Online Registration/‌Change of Address form
  • This communicates your contact information to us
  • New members are still asked to submit the full Parish Membership Registration form (see above)
• Complete and submit the full Parish Membership Registration form (see above)

Updated Registration Information

An existing member may submit a change of address by using the Online Registration/Change of Address form on the Membership Page, or contacting the parish office (see the “Contact Us” page).

Registration for Baptism of an Infant or Young Child

• Printable document
• Fill-in Form
  • E-mail the completed form to Director of Religious Education (or print it out and mail it to or drop off at the parish office).

Contact the parish office regarding the baptism of a teen-ager or adult, or about becoming a Catholic.

Sponsor Form

This form can be used for a sponsor for baptism (a godparent) or confirmation.
